The Parish is required by the Archdiocese to submit all our forms by the end of September. If you have volunteered in any Parish ministry you need to fill out one of these forms and have it verified by a Parish employee.

I will be attending all the Masses on the weekend of September 11 and 12 to pass out the forms. They should be completed at that time and I will verify identification and sign the form. This will make it easier for everyone concerned.

You are only required to fill out a form if you volunteered this past year.
